The adopter agrees to the following terms and conditions.
- (If animal is not spayed/neutered at time of adoption) I agree that I will have the animal spayed/neutered no later than ________________ or six months of age.
- I agree that the animal is being adopted for myself and will not be sold, adopted, or given to another party.
- I agree that the animal will not be used for medical, experimental, laboratory or any other testing.
- (If adopting a cat) I agree that I will not declaw the cat.
- (If adopting a dog) I agree not to crop the ears or dock the tail.
- I agree to care for the animal in a humane manner and be a responsible animal guardian. This includes supplying adequate food, water, shelter, attention, and medical care
- I agree that the animal will be current with all vaccinations and receive regular medical care.
- I agree that unless otherwise approved by WVPR, the principle residence of the pet will be in the home and not outside. If outside facilities are to be used they must meet the approval of WVPR and may not be a chain or similar restraint.
- I agree that if at any point I can not keep the animal, I will return him/her to the WVPR without requesting a fee.
- I understand and agree that WVPR makes no guarantees about the animal's temperament and is not responsible for future damages or injuries caused by the animal
- I give WVPR the right to inspect the animal's home or call my home at any reasonable time, without notice, to assure that the animal is being properly treated and cared for.
- I agree to keep WVPR informed of my current home address and phone number.
I have read and understand and agree to abide by the above terms and conditions of this adoption.
I agree that all statements I have made on this form are true. If it is found that any statements I have made on this form are not true the adopted animal can be confiscated.
